This form is to be used by club executives only for the purpose of registering members that are unable to utilise the online membership application.  The preferred method of registration is via our online membership application.

For those clubs who are unable to utilise the online membership application the following strict guidelines apply due to insurance purposes:

1. All requested details including members date of birth and an email address is mandatory
2. A full and valid postal address recognised by Australia Post
3. Please complete the form, clearly printing (preferably typed) and accurately
4. Do not include Associate members on this form that have already registered with ATHRA through another club
5. A signature is not required if the form is filled in and submitted electronically.
